ABSTRACT

ObjectiveTo investigate the prognostic value of the enhancement pattern in arterial phase of preoperative Gd-EOB-DTPA enhanced magnetic resonance imaging (MRI) in evaluating the disease-free survival (DFS) and overall survival (OS) in patients undergoing curative resection for intrahepatic cholangiocarcinoma (ICC). MethodsA retrospective analysis was done on the clinical, preoperative MRI findings and postoperative follow-up results of 93 pathologically confirmed ICC patients undergoing surgery in our hospital between January 2018 and December 2021. Kaplan-Meier survival curves and log-rank test were used to compare the DFS and OS of three groups with different arterial enhancement patterns. Cox regression analysis was used to identify the factors affecting DFS and OS. ResultsThere were significant differences in DFS and OS among the 3 groups (log-rank test, P < 0.05). The arterial enhancement pattern was an independent predictive factor for DFS (using diffuse hyperenhancement as a reference, peripheral rim enhancement: HR = 3.550; 95%CI: 1.16 ~ 10.8; P = 0.026;diffuse hypoenhancement: HR = 3.430; 95%CI: 1.04 ~ 11.3; P = 0.042). The arterial enhancement pattern and tumor location were predictive factors for OS ((using diffuse hyperenhancement as a reference, diffuse hypoenhancement, HR = 8.500; 95%CI: 1.09-66.3; P = 0.041; using tumor distal location as a reference, tumor perihilar location HR=2.583,95%CI: 1.14-5.83, P =0.022). The AUC of arterial enhancement patterns in predicting 1-, 2-, and 3- year DFS were 0.722, 0.748, and 0.617, respectively; in OS, 0.720, 0.704, and 0.730, respectively, which showed better prognostic efficacy than AJCC-TNM staging system. ConclusionArterial-phase enhancement pattern of preoperative Gd-EOB-DTPA enhanced MRI is an independent predictive factor for DFS and OS of ICC patients, with a better prognostic value than AJCC-TNM staging system, and can be used for the clinical management of ICC patients.

ABSTRACT

OBJETIVO: Determinar las medidas antropométricas y su relación con Enfermedades Crónicas no Trasmisibles en colaboradores de un Call Center del departamento de Managua durante el período Julio- Agosto 2018. DISEÑO: Se realizó un estudio transversal constituido por una muestra de 125 colaboradores de un Call Center del departamento de Managua, el instrumento de recolección de la información estuvo dividida en 3 partes, dedicados a conocer los datos sociolaborales, las medidas antropométricas de índice de masa corporal e índice cintura cadera y las enfermedades crónicas en la población estudiada. RESULTADOS: Predominaron los adultos jóvenes de 21 a 30 años 49.6% (62), de 31 años a 40 años 36 % (45); solteros 60 % (75), de sexo hombre 67.2 % (84); los encuestados presentaron sobrepeso en 39.2 % (49), obesidad grado I en 24 % (30) y obesidad grado II en 6.4% (8). El índice cintura cadera encontrando en hombres y mujeres resultó ser de bajo riesgo y las enfermedades crónicas más frecuentes fueron Hipertensión arterial en un 48.3 % (14), seguida de Diabetes Mellitus en 37.9 % (11). El asma bronquial se presento en 10.3%(3) y cardiopatía en 3.4%(1). La relación del indice de masa corporal con las enfermedades crónicas encontradas en este estudio fue: los individuos con bajo peso no presentaron enfermedades crónicas, el 5.7% (2) presentaron peso normal; 14.3 % (7) se encontraron en sobrepeso,30% (9) estaban en Obesidad Grado I y el 62.5 % (5) en Obesidad II. CONCLUSIONES: De las características sociolaborales la mayoría eran hombres entre las edades de 21-40 años, solteros, con menos de un año de laborar y laboran de 40-48 horas. Más de la mitad de los encuestados presentaron alteración en las medidas antropométricas. Las enfermedades crónicas más frecuentes encontradas fueron la Hipertensión Arterial y Diabetes Mellitus. Según el aumento del perímetro abdominal y grasa corporal expresadas en las medidas antropométricas es posible relacionarlas con las enfermedades crónicas

ABSTRACT

Objective: To evaluate the effect of Houpu Sanwu Tang on the postoperative ileus (POI), and observe its underlying mechanisms of action on interstitial cells of cajal (ICC) and inducible nitric oxide synthase(iNOS) regulation of POI. Method: Totally 87 healthy adult male SD rats were randomly divided into sham operation group, saline control group and Houpu Sanwu Tang group at low, medium and high doses. Houpu Sanwu Tang low, middle and high dose groups received orally Houpu Sanwu Tang(2.25,4.5,9 g·kg-1); Sham operation group (Sham operation) and saline control group (Saline control) received orally normal saline. Surgical procedure was used to induce the postoperative ileus. Changes in intestinal propulsion rate, intestinal mucosal injury and small intestine expression of c-kit and iNOS among these groups were detected. Result: Intestinal propulsion rate was significantly higher in Houpo Sanwu Tang group than that in Saline control group (PPPPConclusion: Houpu Sanwu Tang can improve the intestinal propulsion rate and the recovery in POI rats. The mechanisms are related to the inhibition of the generation of iNOS, the alleviation of inflammatory response, and increase of the number of ICC, so as to ensure its normal function, and improve the intestinal dynamic disorder.

ABSTRACT

Intrahepatic cholangiocarcinoma (ICC) is the second most frequently occurring primary liver cancer. It has been reported that the causes of late diagnosis of ICC are a high degree of malignancy, early metastasis, and diffusion. Most patients visit the hospital because of jaundice or discomfort due to the surrounding compression. ICC commonly recurs after resection, and chemotherapy is not sensitive. Therefore, patients have a poor prognosis and a low survival rate. Currently, a multidisciplinary approach based on surgery is recommended for ICC patients who can undergo surgical excision, and local treatment combined with chemotherapy is the main com-prehensive treatment for patients with advanced ICC who cannot undergo surgical resection. In recent years, it has been found that immunotherapy can involve the autoimmune system to remove tumor cells, and molecular-targeted therapy can kill tumor cells by in-hibiting cell membrane surface molecules that promote tumorigenesis and development. At present, these two treatment modalities have become the research focus of ICC therapy, and progress has been made. The research status is reviewed in this paper.

@#Introduction: The interpretations of printed panoramic radiographs film frequently depend on personal judgements analysis by observers. The findings correlations between more than two observers must consider statistical tool which can determine that sometimes the observers will agree or disagree merely by chance. Intraclass Correlation Coefficient (ICC) generally measures the degree of similarity among individuals within a cluster. Kappa statistic is the most frequently applied statistical analysis for this reason. This study aims to determine the validity of utilising printed panoramic radiographs for assessing dental anomalies and inter-examiner reliability in detecting dental anomalies observed within the panoramic radiographs. Methods: Twenty samples of printed versions of orthopantomogram (OPG) were arbitrarily selected from the AMDI Dental Clinic. Radiographic analyses were conducted twice, Trial 1 and Trial 2, with two-week intervals in between. Ten (10) printed OPG films were used in each session. During both assessments, incidence of dental anomalies was determined. All twenty OPGs were examined and analysed by the principal investigator and two independent examiners according to the inclusion and exclusion criterias. The data analysis was carried out using SPSS version 23. Results: The results of ICC for Trials 1 and 2 were 0.961 and 0.984, respectively. Conclusion: The outcomes demonstrated that the printed OPG films can be used to detect dental anomalies and inter-examiner dependability was in very good assertion for both trials, hence printed OPG films that satisfy the inclusion and exclusion criterias can be utilised for assessing dental anomalies as shown by the reliable ICC values in this study.

BACKGROUND: Fine-needle aspiration cytology (FNAC) is still an important first line diagnostic procedure in developing countries. FNAC of breast lesions is quite specific and sensitive investigation and the results are comparable to histopathology.Aim of study is to evaluate role of fine needle aspiration cytology in the diagnosis of breast lesions and to compare the cytological findings with the histopathology wherever possible. In addition the sensitivity, specificity and accuracy of FNAC in breast lesions are carried out. MATERIAL METHODS &RESULT: The present study was carried out on 100 patients who presented with palpable lump in the breast ;irrespective of age and sex were included in the study from the district hospital of North Karnataka, over a period of 15 months.In 69 cases ,cytological diagnosis was correlated with histological findings . RESULTS: There was preponderance of benign breast lesions: 55cases (79.71%) and malignant cases 14(20.29%) .In present study,sensitivity is 92.85%,specificity is 100% in diagnosisng malignant lesions,with accuracy of 98.48% .Positive predictive value is 100% and negative predictive value is 98.11%. CONCLUSION: With experienced hands, FNAC is safe, cost effective and a reliable technique for preoperative evaluation of palpable breast lumps. FNAC features are more informative when combined with physical and radiology findings (Triple test).

ABSTRACT

<p><b>OBJECTIVE</b>To compare the effects of electroacupuncture (EA) at "Dachangshu" (BL 25) or "Tianshu" (ST 25) for visceral sensitivity, gene expression product c-kit of colonic Cajal interstitial cells (ICC) and capsaicin receptor 1 (TRPV1) of irritable bowel syndrome (IBS) rats, so as to investigate the effect and mechanism differences of EA at the back point and the front point of large intestine for IBS rats.</p><p><b>METHODS</b>Forty-two Wistar neonatal rats were randomly divided into a blank group (9 rats) and a model group (33 rats). IBS model was established with mother and child separation, acetic acid enema in young rats and colorectal dilatation method. Twenty-seven IBS rats in life were randomly divided into a model control group, a Dachangshu group and a Tianshu group, 9 rats in each group. EA (disperse-dense wave, 2 Hz/100 Hz, 0.1-0.3 mA) for 20 min was used at "Dachangshu" (BL 25) and "Tianshu" (ST 25) respectively in the Dachangshu and Tianshu groups, once every other day, totally 5 times. The rats in the model control group were fixed with soft cloth sleeve for 20 min, without acupuncture. No intervention was used in the blank group. The stool property Bristol grading score was recorded before and after intervention in each group. The visceral sensitivity was evaluated by abdominal withdrawal reflex. The latency until the first systolic wave occurred and the number of systolic wave within 90 s were observed. Immunohistochemical was used to detect the positive expressions of c-kit and TRPV1, the ICC colon specific marker.</p><p><b>RESULTS</b>Compared with the blank group, the Bristol score increased,latency period shortened, systolic wave number increased, c-kit and TRPV1 positive expressions increased in the model control group (all <0.01). Compared with the model control group, the Bristol score decreased, latency period increased, systolic wave number decreased, c-kit and TRPV1 positive expressions decreased after intervention in the Dachangshu and Tianshu groups (<0.05, <0.01). Compared with the Dachangshu group, the TRPV1 positive expression decreased after intervention in the Tianshu group (<0.05).</p><p><b>CONCLUSION</b>EA at "Dachangshu" (BL 25) or "Tianshu"(ST 25) can improve the diarrhea in IBS model rats, reduce the visceral sensitivity, and its mechanism may be related to regulating the expressions of colon c-kit and TRPV1. EA at "Tianshu" (ST 25) is more apparent for TRPV1 than at "Dachangshu" (BL 25).</p>

ABSTRACT

La diabetes es una enfermedad crónica degenerativa, exponencialmente creciente, y generando altos costos para los sistemas de salud a nivel mundial. Un alto porcentaje de la población utiliza medicina tradicional como extractos de plantas para satisfacer sus necesidades de atención primaria de salud. El objetivo del estudio fue desarrollar y evaluar una infusión de té rojo con moras enriquecido con ß-glucanos para el control de glicemia en personas diabéticas de la aldea de Jicarito. Se utilizó un DCA con un arreglo factorial 2 × 2 × 2, evaluando dos concentraciones de té rojo con mora (50/50 y 75/25), dos temperaturas (75 y 95 °C) y dos tiempos (2 y 5 minutos), seleccionando el tratamiento con mayor contenido de polifenoles totales (P2T2t2). El mejor tratamiento se evaluó en una muestra de 30 diabéticos, a los cuáles se realizaron medidas antropométricas (peso, talla, circunferencia de cintura-cadera y presión arterial), medidas bioquímicas (glucosa y colesterol) y se analizó la ingesta alimentaria a través de recordatorio de 24 horas. Los resultados demostraron reducción de Índice de Masa Corporal (IMC) en edades de 44- 65 años, en niveles de glucosa en edades de 45-65 y > 65 años, en niveles de colesterol total en edades de 45-65 años y no existió reducción en presión arterial ni en el Índice de Cintura- Cadera (ICC) para ambos sexos. Se determinó una mayor ingesta alimentaria de hidratos de carbono y grasas en la población de diabéticos. Para evaluar modificaciones en medidas antropométricas y bioquímicas se necesita de al menos tres meses de consumo del té.

Objective To investigate the spatio-temporal characteristics of normal gastric electrical activity.Methods From January 1st to February 29th,2016,fasting gastric electric slow wave signal was collected by high resolution mapping (HRM) under anesthesia before operation in patients without gastric lesions but needed laparoscopic surgery.Parameters of slow wave signal activity was calculated,gastric activity map and velocities was drawn and then spatio-temporal characteristics of gastric electrical activity of different part of the stomach were obtained.Independent sample t test was performed for comparison of two groups and one-way analysis of variance was used for multiple group comparisons.Results The normal gastric pacing zone located in the upper middle part of gastric body near greater curvature.Electrical activity of gastric body spread to gastric antrum along the long axis of the stomach.No slow signal was recorded in the gastric fundus and the gastric cardia.The frequencies of slow waves of different parts of gastric was same and the mean value was 2.61 ±0.11 cycle per minute.Compared with that of gastric body,the slow wave amplitude of gastric pacing zone was higher ((4.19±0.73) mV vs (1.67± 0.89) mV) and the speed was faster (7.24± 1.37) mm/s vs (4.94± 0.20) mm/s);the differences were statistically significant (t=18.89 and 4.95,both P<0.01).The slow wave amplitude of gastric antrum was higher than that of gastric body ((3.21±0.49) mV vs (1.67±0.89) mV) and the speed was also faster ((6.44±0.82) mm/s vs (4.94±0.20) mm/s);the differences were statistically significant (t=4.85 and 4.95,both P<0.05).The slow wave amplitude of gastric antrum was lower than that of pacemaker area ((3.21±0.49) mV vs (4.19±0.73) mV),and the difference was statistically significant (t =-3.67,P<0.05);however,there was no significant difference in wave velocity ((6.44±0.82) mm/s vs (7.24±1.37) mm/s,P>0.05).Conclusions The normal human gastric pacemaker is located in mid and upper corpus near the greater curvature,which produces slow wave and control whole gastric electrical activity.The amplitude and velocity of slow waves are in gradient changes in different gastric regions.HRM is a mature and reliable research method to study the spatio-temporal characteristics of gastric electrical activity,which provides the possibility for the study of abnormal gastric electrical activity.

ABSTRACT

OBJECTIVE: To observe the effects of electroacupuncture (EA) on gastrointestinal motility and the ultrastructure of interstitial cells of Cajal (ICC) and the expressions of c-kit receptor protein and stem cell factor (SCF) mRNA in diabetic gastroparesis (DGP) rats, so as to explore its mechanism. METHODS: Fifty SD rats were randomly divided into normal, model, acupoint, non-acupoint and metoclopramide groups (n=10 rats/group). DGP model was established by intraperitoneal injection of streptozotocin (STZ, 2%), and raised with high-sugar high-fat diet irregularly. EA (sparse-dense, 10 Hz/50 Hz, 2 mA, 20 min) was applied at "Zusanli" (ST 36), "Liangmen" (ST 21) and "Sanyinjiao" (SP 6), and the corresponding non-acupoints of the 3 acupoints, daily for 15 days. The rats in metoclopramide group received intragastric administration of metoclopramide (1.7%, 1 mL/100 g) for 15 days, once a day. Blood sugar was determined with One Touch blood glucose test paper. The gastric emptying rate (GER) and the intestinal propulsion rate (IPR) were measured by intragastric phenol red. The ultrastructure of ICC was detected by transmission electron microscopy. The expression levels of c-kit receptor protein and SCF mRNA of gastric antrum were examined respectively by Western blot and RT-PCR. RESULTS: Compared with the normal group, the blood glucose significantly increased in the model group (P<0.01), while the GER, IRP and the expression level of SCF mRNA in the gastric antrum significantly decreased (P<0.01), and the ultrastructure of ICC appeared apoptosis-like changes. The blood glucose of the EA group was obviously decreased compared with that of the model group (P<0.05); the GER and IRP significantly increased(P<0.05, P<0.01); the expression level of SCF mRNA increased (P<0.01), the number of ICC increased and its ultrastructure was repaired. There was some relief on ICC ultrastructure in the acupoint group compared with that in the non-acupoint group; and SCF mRNA increased (P<0.05). There was no significant difference on c-kit receptor expression among all the modeling groups (P>0.05). CONCLUSIONS: EA at ST 36, etc. can regulate the blood glucose and improve gastrointestinal emptying in DGP rats. The mechanism may be related to up-regulating SCF mRNA, repairing ICC ultrastructure, restoring the pacing function, and improving gastrointestinal motility.

ABSTRACT

This study aimed to investigate the effect of pituitary adenylate cyclase-activating peptide (PACAP) on the pacemaker activity of interstitial cells of Cajal (ICC) in mouse colon and to identify the underlying mechanisms of PACAP action. Spontaneous pacemaker activity of colonic ICC and the effects of PACAP were studied using electrophysiological recordings. Exogenously applied PACAP induced hyperpolarization of the cell membrane and inhibited pacemaker frequency in a dose-dependent manner (from 0.1 nM to 100 nM). To investigate cyclic AMP (cAMP) involvement in the effects of PACAP on ICC, SQ-22536 (an inhibitor of adenylate cyclase) and cell-permeable 8-bromo-cAMP were used. SQ-22536 decreased the frequency of pacemaker potentials, and cell-permeable 8-bromo-cAMP increased the frequency of pacemaker potentials. The effects of SQ-22536 on pacemaker potential frequency and membrane hyperpolarization were rescued by co-treatment with glibenclamide (an ATP-sensitive K+ channel blocker). However, neither N(G)-nitro-L-arginine methyl ester (L-NAME, a competitive inhibitor of NO synthase) nor 1H-[1,2,4]oxadiazolo[4,3-alpha]quinoxalin-1-one (ODQ, an inhibitor of guanylate cyclase) had any effect on PACAP-induced activity. In conclusion, this study describes the effects of PACAP on ICC in the mouse colon. PACAP inhibited the pacemaker activity of ICC by acting through ATP-sensitive K+ channels. These results provide evidence of a physiological role for PACAP in regulating gastrointestinal (GI) motility through the modulation of ICC activity.

ABSTRACT

Background: The role of fi ne needle aspiration cytology (FNAC) in the diagnosis of malignant lesions is well documented. Conventionally obtained fi ne needle aspiration (FNA) smears frequently contain well preserved viable intact tissue fragments (microbiopsies). Aims: The present study was aimed at evaluating the microbiopsies and carrying out further cyto-histopathologic correlation in order to assess what additional information they could provide. Materials and Methods: A total of 116 FNA smears from clinically suspected malignant lesions were examined, of which 81 smears (70.0%) contained representative tissue fragments of the tumors. Histopathological details were available in 75 cases. Immunocytochemistry (ICC) was applied in selected smears as and when required. Results: Tumors in which microbiopsies aided in diagnosis chiefl y included soft tissue malignancies (12 cases), typed into malignant peripheral nerve sheath tumor, fi brosarcoma, spindle cell sarcoma and malignant melanoma; lung tumors (14 cases) classifi ed as small cell carcinoma, large cell carcinoma, squamous carcinoma and adenocarcinoma; breast tumors (08 cases), typed into ductal, medullary and lobular carcinoma; and lymph node metastasis (19 cases) from primaries in lung, larynx and thyroid. The cyto-histopathologic concordance was raised from 81.2% in the absence of microbiopsies to 93.2% in their presence. Conclusions: FNA smears containing microbiopsies are of ample help in establishing fi rm diagnosis, tumor typing, and predicting possible primary sites in metastatic tumors which were not possible by cytology alone. Hence, this technique can be utilized to enhance the diagnostic accuracy of FNAC, if put into practice in evaluation of routine cytology smears, without increasing any fi nancial burden on patients.

ABSTRACT

Indian childhood cirrhosis (ICC), a disease considered to have been endemic in and unique to India has now been documented in children of non-Indian origin from other countries. More recently available findings from a large multicentre study sponsored by the Indian Council of Medical Research (ICMR) have dispelled some of the generally accepted notions and have established several new facts on different aspects of the disease. All relevant reports on ICC and ICC-like diseases, till date, were reviewed to obtain a proper perspective on the current state of our understanding on this non-Wilsonian copper overload liver disease. A primary role of exogenous copper in causing the disease was earlier debated on the basis of studies in India but investigators abroad studying some sporadic cases and a series of endemic ICC-like diseases supported a hepatotoxic injury by ingested copper in genetically susceptible infants and children in ICC- like disease and in ICC. Epidemiologic and morphologic findings in the well controlled ICMR study based on 225 cases of ICC and 426 controls, all confirmed on liver biopsy, have however, convincingly refuted this concept. Additionally, this study revealed that unlike what has been believed earlier, older children more than 3 yr age can get the disease and that in its natural course the hepatic histology can transform between the characteristic one considered diagnostic and some other patterns, any one of which can be the morphologic manifestation at first presentation of the patient. Older children and cases with milder morphologic changes at presentation had longer survival. The overall inference from critical analysis of all available data is that ICC and ICC-like diseases clinically manifest in a child of any age though common in younger ones, and a clinical diagnosis must be made in any child with so-called ‘cryptogenic cirrhosis’. Exposure to exogenous copper in food, milk and water should not be a prerequisite for this consideration. A liver biopsy whenever feasible should be mandatory for confirmation with the understanding that the morphologic changes in liver can present a few other patterns besides the characteristic one currently taken to be diagnostic. The ascribed current decline in encountering ICC is likely to be due partly to missing a diagnosis and partly to a true reduction in incidence consequent on time related economic and socio-cultural changes.

Este artigo avalia a confiabilidade teste-reteste do Índice de Capacidade para o Trabalho (ICT) em trabalhadores de enfermagem. Foi aplicado questionário autopreenchível duas vezes a um grupo de 80 trabalhadores de enfermagem (enfermeiros, técnicos e auxiliares de enfermagem) de um hospital público no Município do Rio de Janeiro, com intervalo entre sete e quinze dias. A confiabilidade foi estimada pela estatística kappa ponderada quadrática, pelo coeficiente de correlação intraclasse (CCI) e pelo gráfico de Bland e Altman. Dos participantes, 81% eram mulheres e com idade variando de 22 a 67 anos (média = 39,1; DP = 10,8anos) e 36,3% tinham ensino superior completo. O escore global do ICT apresentou CCI = 0,79 (IC95% 0,67 a 0,86) e kappa = 0,69 (IC95% 0,50 a 0,80) para o ICT categórico (classificado em baixo, moderado, bom e ótimo). O kappa dos itens do ICT variou de 0,39 a 0,82 e o gráfico de Bland e Altman não mostrou um padrão sistemático. A concordância entre as medidas de teste-reteste indica grau aceitável de confiabilidade, sugerindo adequação do processo de aferição entre trabalhadores de enfermagem.


This paper assesses the test retest reliability of the Work Ability Index (WAI) in nursing workers. A self-administered questionnaire was applied twice to a group of 80 workers (nurses and nursing aides/assistants) at a public hospital in Rio de Janeiro, Brazil within an interval from seven to fifteen days. The reliability was estimated using quadratic weighted kappa statistics, interclass correlation coefficient (ICC) and the Bland and Altman plot. Eighty-one percent of participants were women aged betweenfrom 22 to 67 years (mean =39.1; SD=10.8 years); 36.3% had completed higher education. The global score of the WAI presented ICC= 0.79 (IC95% 0.67 to 0.86) and weighted kappa=0.69 (CI95% 0.50 to 0.80) for categorical WAI (classified as low, moderate, good and excellent). The quadratic weighted kappa of the WAI items ranged from 0.39 to 0.82 and the Bland and Altman plot did not show a systematic pattern. The agreement between the test and retest measures shows an acceptable degree of reliability, suggesting the adequacy of the assessment process among nursing workers.

ABSTRACT

Menopause is associated with an increased body weight and changes in fat distribution, high levels of homocysteine and cardiovascular risk factors associated with estrogen deficiency. The objective was to evaluate body mass index (BMI), waist circumference (WC), waist/hip index (WHI), and serum homocysteine (Hct) levels in postmenopausal women (n 128). Nutritional status was diagnosed by BMI (WHO), WC (normal <88cm, at risk> 88cm), WHI (normal <0.8, at risk> 0.8), serum homocysteine (Hct) (ELISA) normal < 10 mmol/L, at risk >10-15mmol/L, high >15mmol/L and estradiol (ELISA) <65pg/mL (menopause). Sixty five point nine percent were overweight/obese, 47.3 percent and 82.2 percent showed cardio metabolic risk by WC and WHI. There was a significant difference for WHI, and a positive significant correlation between anthropometrics indexes. Ten percent showed risk and hyperhomocysteinaemia, but it was not correlated with the evaluated variables. The subjects had a high frequency of overweight, obesity and android fat distribution, showing a high risk for cardiometabolics diseases.


La menopausia se asocia a un aumento del peso corporal y a cambios en la distribución de grasa, describiéndose también niveles elevados de homocisteína, factores de riesgo cardiovascular asociados al déficit de estrógenos. El objetivo de este estudio fue evaluar índice de masa corporal (IMC), la circunferencia de cintura (CCi), el índice cintura-cadera (ICC), la homocisteína sérica en mujeres posmenopáusicas (n: 128). El diagnóstico nutricional antropométrico se determinó según IMC (OMS); se determinaron la CCi (normal <88cm, en riesgo >88cm), ICC (normal <0,8, en riesgo >0,8), homocisteína sérica (tHci) (ELISA): normal < 10mmol/L, en riesgo >10-15mmol/L, alta: >15mmol/L y estradiol (ELISA): <65pg/mL (posmenopausia). 65,9 por ciento presentaron sobrepeso/ obesidad. 47,3 por ciento y 82,2 por ciento en riesgo cardiometabólico según CCi e ICC. Hubo diferencia significativa para ICC y correlación positiva significativa entre indicadores antropométricos. 10 por ciento presentó riesgo e hiperhomocisteinemia. Este aminoácido no correlacionó con las variables evaluadas. Las mujeres evaluadas presentaron una alta frecuencia de sobrepeso-obesidad y una distribución de grasa tipo androide, presentando un alto riesgo para enfermedades cardiometabólicas.

ABSTRACT

0 aumento significativo do número de casos notificados da Leishmaniose Tegumentar Americana (LTA) e a expansão geográfica da endemia tem motivado o desenvolvimento de novas tecnologias para auxiliar no diagnóstico das leishmanioses, visando minimizar as restrições apresentadas pelos testes diagnósticos disponíveis nos serviços de saúde. 0 presente trabalho empregou imunocitoquimica e imunohistoquímica (ICQ/IHQ) como métodos diagnósticos laboratoriais para LTA. Amostras de culturas de Leishmania in vitro e cortes histológicos de lesões em animais infectados experimentalmente foram submetidos a ICQ/IHQ, utilizando anticorpos policlonais desenvolvidos para este estudo e o complexo avidina-biotina modificado (Ultra Streptavidin®). Em comparação com outras técnicas empregadas para o diagnóstico da LTA, nos casos avaliados, a IHQ apresentou resultados semelhantes aos da histopatologia com coloração HE, com sensibilidade de 33,3% para formas amastigotas. Quando considerada a presenva de antígenos de Leishmania no padrão celular, a IHQ apresentou uma sensibilidade de 83,3%, significativamente maior que na histopatologia e compatível com metodos padrão ouro de cultura e PCR. As metodologias de ICQ/IHQ desenvolvidas neste trabalho foram capazes de demonstrar em biópsias de lesões, a presença de formas amastigotas e antígenos de Leishmania, oferecendo contribuição adicional ao diagnóstico da LTA, sendo de facil aplicação e podendo ser utilizada no sistema público de saúde.


The significant increase in cutaneous leishmaniasis (CL) notified cases and the geographic expansion of this endemy has motivated the development of new techniques to help in leishmaniasis diagnosis, seeking the minimization of the restrictions imposed by the diagnostic tests available at the health services. The current study applied immunocytochemistry and immunohystochemistry methods (ICC/IHC) for laboratory diagnosis of CL. Imprints and histological sections from tissue infected with Leishmania were submitted to ICC/IHC methods using polyclonal antibodies developed for this study and a modified avidin-biotin complex (Ultra Streptavidin®). The samples also were submitted for routinely stained hematoxylin and eosin (H&E) specimens and gold standard methods (culture and PCR). Compared with other useful techniques for the CL diagnosis, ICC/IHC showed the same sensitivity results (33%) as H&E stain for amastigotes recognition. When the presence of Leishmania antigens was evaluated, ICC/IHC presented 83,3% sensitivity, i.e., higher than that detected by histopathology and equivalent with gold standard methods (culture and PCR). The ICC/IHC techniques developed in the current study were able to recognize amastigote forms and also Leishmania antigens in lesion biopsies, offering an additional help to CL diagnosis and it can be easily applied in the public health system.

ABSTRACT

We studied whether nitric oxide (NO) and hydrogen sulfide (H2S) have an interaction on the pacemaker activities of interstitial cells of Cajal (ICC) from the mouse small intestine. The actions of NO and H2S on pacemaker activities were investigated by using the whole-cell patch-clamp technique and intracellular Ca2+ analysis at 30degrees C in cultured mouse ICC. Exogenously applied (+/-)-S-nitroso-N-acetylpenicillamine (SNAP), an NO donor, or sodium hydrogen sulfide (NaHS), a donor of H2S, showed no influence on pacemaker activity (potentials and currents) in ICC at low concentrations (10 microM SNAP and 100 microM NaHS), but SNAP or NaHS completely inhibited pacemaker amplitude and pacemaker frequency with increases in the resting currents in the outward direction at high concentrations (SNAP 100 microM and NaHS 1 mM). Co-treatment with 10 microM SNAP plus 100 microM NaHS also inhibited pacemaker amplitude and pacemaker frequency with increases in the resting currents in the outward direction. ODQ, a guanylate cyclase inhibitor, or glibenclamide, an ATP-sensitive K+ channel inhibitor, blocked the SNAP+NaHS-induced inhibition of pacemaker currents in ICC. Also, we found that SNAP+NaHS inhibited the spontaneous intracellular Ca2+ ([Ca2+]i) oscillations in cultured ICC. In conclusion, this study describes the enhanced inhibitory effects of NO plus H2S on ICC in the mouse small intestine. NO+H2S inhibited the pacemaker activity of ICC by modulating intracellular Ca2+. These results may be evidence of a physiological interaction of NO and H2S in ICC for modulating gastrointestinal motility.

ABSTRACT

In this study we determined whether or not 5-hydroxytryptamine (5-HT) has an effect on the pacemaker activities of interstitial cells of Cajal (ICC) from the mouse small intestine. The actions of 5-HT on pacemaker activities were investigated using a whole-cell patch-clamp technique, intracellular Ca2+ ([Ca2+]i) analysis, and RT-PCR in ICC. Exogenously-treated 5-HT showed tonic inward currents on pacemaker currents in ICC under the voltage-clamp mode in a dose-dependent manner. Based on RT-PCR results, we found the existence of 5-HT2B, 3, 4, and 7 receptors in ICC. However, SDZ 205557 (a 5-HT4 receptor antagonist), SB 269970 (a 5-HT7 receptor antagonist), 3-tropanylindole - 3 - carboxylate methiodide (3-TCM; a 5-HT3 antagonist) blocked the 5-HT-induced action on pacemaker activity, but not SB 204741 (a 5-HT2B receptor antagonist). Based on [Ca2+]i analysis, we found that 5-HT increased the intensity of [Ca2+]i. The treatment of PD 98059 or JNK II inhibitor blocked the 5-HT-induced action on pacemaker activity of ICC, but not SB 203580. In summary, these results suggest that 5-HT can modulate pacemaker activity through 5-HT3, 4, and 7 receptors via [Ca2+]i mobilization and regulation of mitogen-activated protein kinases.
